By carefully positioning cortical screws in the special holes in compression plates, compression can be exerted at the reduced fracture site as the screw is tightened. As the head of the screw strikes the edge of the plate’s hole, the screw (and the associated bone) are forced away from the edge of the hole and toward the fracture site. The object of compression is to provide for primary bone union. A special guide must be used to position the screw hole accurately in the plate. Holes for cortical screws must be pre-tapped prior to screw placement.
